Hello,
My name is _____________, I'm with the United States Patent and Trademark Office.
You recently completed one of our agency's courses through the StART program. In
an effort to improve our training courses, I'd like to ask you a few questions regarding
your efforts to file a patent application and how the course impacted those efforts. It
should take no more than 20 minutes, and your responses will be kept confidential
and all data will be analyzed in aggregated form only.
If that's ok we will proceed with the survey.

A Federal agency may not conduct or sponsor, and a person is not required to respond to, nor shall a person be subject to a penalty for

burden for this survey is estimated to average 20 minutes per response, including the time for reviewing instructions, searching existing
