Introduction
The Urban Institute, an independent research organization based in Washington, D.C., is conducting an annual survey as part of the Pay for Success (PFS) initiative in <Site Name>. This survey is part of an evaluation of the U.S. Department of Housing and Urban Development (HUD) and the Department of Justice PFS Initiative.
What is the purpose of the survey?
You are being asked to participate in this survey because of your role in an organization that is involved with the <Site Name>’s Pay for Success Initiative. The survey collects information on:
how partner organizations in the initiative and the broader community collaborate,
data utilization and sharing across sectors, and
innovation in service delivery.
Since this is intended to capture changes in <Site Name> over time, the questions asked in the survey remain the same each year.

How will my answers be shared?
Participation in the survey is voluntary. Only aggregate responses will be publicly reported – never individual responses. However, due to the nature of the project, a unique role or response could be potentially identifying. Responses will be kept private to the extent permitted by law.
