and Program Data Report under Approved OMB
Clearance 0584-0594

The Food and Nutrition Service (FNS), Supplemental Food Programs Division is
requesting approval to change the form number from FNS 683 to FNS 683B WIC
Farmers’ Market Nutrition Program Annual Financial and Program Data Report. The
original FNS 683 needs to remain in FPRS for historical purposes. The 683 B will be
implemented in FY 2016. This 683 B is necessary because it combined data elements
from FNS-203 which was approved by OMB 10/1/2014.
The reporting burden for the FNS 683B will be contained in the Food Programs
Reporting System (FPRS) OMB Control No. 0584-0594, Expiration Date: 6/30/2017.
