The Paperwork Reduction Act of 1995: This information is collected in accordance with section 3507 of the Paperwork Reduction Act of 1995. Accordingly, we may not conduct or sponsor and you are not required to respond to, a collection of information unless it displays a valid OMB number. We anticipate that the time expended by all individuals who complete this survey will average 10 minutes. This includes the time it will take to follow instructions, gather the necessary facts and respond to questions asked. Customer satisfaction is used to gauge customer perceptions of VA services as well as customer expectations and desires. The results of this telephone/mail survey will lead to improvements in the quality of service delivery by helping to achieve improved mental health services. Participation in this survey is voluntary and failure to respond will have no impact on benefits to which you may currently be receiving.

Informed Consent - Electronic Survey
Welcome to the Southern Oregon University and Southern Oregon Rehabilitation Center and Clinics program offering survey. Brad Christ of the Information Technology department at Southern Oregon University is conducting this research. The results of the survey will provide important information about your university experience and help create a better environment for working and learning.
No personal information will be collected. All information is strictly confidential and will be protected to the degree permitted by the technology used (e.g. IP addresses will be stripped when the survey is submitted).
Only group data will be reported, meaning the analysis will include only aggregate data. Results will be statistically compiled. No names will ever be used in any report of the results of this study. The aggregate data will be analyzed and may be presented at professional conferences.
