In a few days, you will receive the Identity Protection PIN Customer Experience Survey asking about your experiences with the IP PIN. Introduction
This survey has been sent to you because our records indicate you have received an Identity Protection Personal Identification Number (IP PIN) for use with your 2012 federal tax return. The IP PIN was provided in a letter sent to you from the Internal Revenue Service (IRS).
The IP PIN is a six-digit identification number sent to taxpayers who have reported to the IRS that they have been the victim of identity theft and have provided information to the IRS to prove their identities. The IP PIN was provided for use when filing tax returns to indicate the filer is who they claim to be.
The purpose of this survey is to learn about your experiences involving the IP PIN to help the IRS improve services to taxpayers who have experienced tax related identity theft issues. Your responses are voluntary and there are no penalties if you choose not to participate.
This survey is not a review of your tax records. Your responses are important to the accuracy and usefulness of the overall results and will help the IRS improve its services.
