TITLE OF INFORMATION COLLECTION:

NRC Form 659, Public Meeting Feedback Form
PURPOSE:

As part of NRC's effort to implement its Strategic Plan performance goal on openness, the public meeting feedback form will be used
to assess the effectiveness of NRC's communications and outreach with the public. The form is intended to be a qualitative method of
collecting information, which will be used to identify areas where NRC staff can improve its public interactions.

DESCRIPTION OF RESPONDENTS:

Respondents will be participants at NRC public meetings, who will reply using one of the following methods: (1) a business reply
mailer, which can either be filled out at the meeting or returned by mail to NRC; (2) an electronic form that can be found on the NRC
website; or (3) a phone app which will be piloted during FY2013.

INSTRUCTIONS FOR COMPLETING NRC FORM 671

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the
request. (e.g. Comment card for soliciting feedback on xxxx)
PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part
of a larger study or effort, please include this in your explanation.
DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this
collection of information. These groups must have experience with the program.
