This survey asks about what you eat. This study is being sponsored by the U.S. Department of Agriculture’s Food & Nutrition Service and conducted by RTI International, a non-profit research organization. The survey will take about 15 minutes to complete. You will receive $10 for completing this survey and $15 for filling out the second survey in about a month. All
of your answers to the survey will be kept private. We will not
share your answers with anyone, except as otherwise required by
