The following questions have been updated since the last OMB Approval:

The FEMA Form numbers has changed on the following forms:

FEMA Form 089-0-24 (previously FEMA Form 90-133), FEMA Form 078-0-1 (previously FEMA Form 90-58), and FEMA Form 078-0-2 (previously FEMA Form 90-32)

Supporting Statement:

The # of respondents increased from 18 to 25.

The total burden hours increased from 334 hours to 810.5 hours. (Note: The numbers in ROCIS are different due to rounding and total 811 in ROCIS.)
