None of the proposed changes below affect the burden hours for the Information Collection. These changes replace a transmission method and correct an error to a website link.

Change:

Change the website link on page 5 (“Data Specifications”) and page 8 (“Mail Your Completed Selection Form To:”) of the FIDM Election Form Instructions from:

http:/www.acf.dhhs.gov/programs/cse

UPDATED TO:

http://www.acf.hhs.gov/programs/cse/

Change:

On page 5 (“Types of Media”), page 7 (“Media”), and page 9 of the Election Form (“Media”) the option for “CONNECT:Direct” needs to be removed, as this transmission method is no longer available. The new transmission method option is “Secure File Transfer Protocal.”
