This generic
clearance for FDA customer service surveys is approved consistent
with the revised supporting statement of (5/25/05) under the
following conditions: 1) FDA shall use the generic clearance only
for customer satisfaction and web site user surveys where FDA seeks
to gather information for general service improvement, not for
publication or for the purpose of informing significant policy or
resource allocation decisions. 2) FDA shall submit an 83-C along
with an abbreviated supporting statement for individual surveys
(e.g., statement of need, intended use of information, description
of respondents, information collection procedures, expected
response rate, justification for incentive, estimated burden). 3)
OMB will respond with clearance or questions within 10 working
days.

This agreement will allow FDA to
conduct customer satisfaction surveys to gain important feedback
from regulated entitites such as food processors, cosmetic, drug,
biologic, and medical device manufacturers as well as consumers and
health professionals and partner surveys of the State and local
governments.
On behalf of this Federal agency, I certify that

1320.8(b)(3).
