tentatively expecting a new submission in the near future that
would create a new SSA form 3821. This new form will be identical
to theSSA-3820, with the addition of two questions from the current
SSA-454 BK.

SSA requires claimants to furnish
medical and other evidence to prove they are disabled. The form
SSA-3820-BK is used to obtain various types of information about a
child's condition, his/her treating sources and/or medical sources
of evidence. The informaiton collected on the SSA-3820-BK is needed
for the determination of disability by the State Disability
Determination Services. The respondents are applicants for Title
XVI Child disability benefits.
