The information collected on form
SSA-3820 is needed for the determination of disability by the State
Disability Determination Services. The SSA-3820 will be used to
develop medical evidence and assess the alleged disability of a
child filing for eligibility under title XVI. The information,
together with medical evidence, forms the evidentiary basis upon
which the initial disability evaluation process is
founded.
