The Substance Abuse and Mental Health Services Administration (SAMHSA) is requesting a no material or non-substantive change approval from the Office of Management and Budget (OMB) for (OMB No. 0930-0401) the Evaluation of the Zero Suicide in Health Systems Program (ZSE).


SAMHSA is requesting a non-substantive change to revise questions to align with E.O. 14168 Defending Women From Gender Ideology Extremism and Restoring Biological Truth to the Federal Government.


SAMHSA is requesting the following changes from attachments B, D, E, H, I, J, K and L in the “Evaluation of the Zero Suicide in Health Systems Program (ZSE)”. These changes will not impact the current burden hours:


  • Replace the question “Which of the following do you consider yourself to be” with “What is your sex”.

  • Maintained only the response options “female” and “male” and remove all response options for the question on “sex”.

  • Removed all other subsequent questions related to “sexual orientation” and “gender identity”.

  • Remove the word “culture” in some places and replace the word “culture” with “personal attributes” or “targeted” where applicable.

  • Replace the words “diverse and underserved populations” with “population sub-groups”.

  • Replaced one of the response options “People with lived experience from diverse racial, ethnic, and sexual gender minority groups” with “People with lived experience from various demographic groups” for the question 3 in the prevention strategies inventory tool.

  • Replaced the word “diverse” with “different”.

  • Remove one of the response options “Lesbian, gay, bisexual, and transgender (LGBTQI+) populations” for questions 18, 29, 40, 51, 62 in the prevention strategies inventory tool.

  • Removed a response option “Language or cultural considerations (CLAS standards)” for the question 20, 31, 42, 53, 64 in the prevention strategies inventory tool.

  • Replaced the word “survey” with “interview”.

  • Removed the question 9 (d) “Next, I’d like to ask you to think about the parts of your culture that are important to your identity, or who you are as a person. This can include race, gender, ethnicity, age, social class, disability status, size, religion, sexual orientation, and many other cultural factors. How did your care team incorporate elements of your culture or identity into your treatment?” in the “Consumer key informant interview guide”.

  • Removed question 19 in section 3 of “Consumer Experience Survey”.

  • Removed the question 1 (C) “Culturally adapted approaches to suicide prevention” in section 1 of “training utilization and preservation baseline”.

  • Reworded the question 11 in “Key informant interview guide cost sub-study” from “How does your organization address behavioral health equity in the context of the Zero Suicide program? Please discuss any strategies or practices you have implemented to ensure that suicide prevention services are accessible and effective across diverse and underserved populations, including any specific adaptations made to cater to the unique needs of these groups” to “How does your organization work to improve the availability and appropriateness of suicide prevention services within your service population? Please discuss any strategies or practices your organization has implemented to ensure that suicide prevention services are accessible and effective across populations”.

  • Removed a response option “People with lived experience from diverse racial, ethnic, and sexual gender minority groups” for the question 3 in part 1 of “Prevention strategies inventory” survey.
