Addendum to Generic Information Collection Request

Usability and Cognitive Evaluation for

the Data Collection Instrument of FoodAPS-2 Pilot Study

The U.S. Census Bureau would like to include an addendum to the ICR package for the usability and cognitive evaluation of the data collection instrument for the Second National Food Acquisition and Purchase Survey (FoodAPS-2) Pilot Test, under the generic clearance for questionnaire pretesting research (OMB number 0607-0725, ICR Reference Number: 202209-0607-002). The following are the items to be revised:

1. Replace the age range of 11-15 years old with 13-15 years old for school children to self-report food acquisition. See page 1, 2, 3, 4 in the document “OMB request for FoodAPS-2 Pilot Test usability evaluation revision 20240212.” The changes are highlighted in yellow.

2. Added two questions and revised two original questions in the screener to collect specific age ranges of potential participants. See page 2 in the document “Appendix D Screener Questionnaire for Participant Recruitment revision 20240212.” The changes are highlighted in yellow.
