Accessing the AD-755 Form
Access to Electronic form is via a USDA directing users to the USDA DASO Now Platform.
Subnavigation to the AD-755 form is accessible only through successful login using DASO
identity access management application.

Form Content Delivery - Presented via a stepper interface, using successive pages to capture
and conditionally display form fields for completion, each step is available once the preceding
step’s required fields are completed. Instructions concerning required fields are provided on
each page and may also include supplemental questions.
