Privacy Act Statement Privacy Act Statement (5 U.S.C. § 552a(e)(3))
Authority: The authority for collecting this information on FAA 8130-1, Application for Export
Certificate of Airworthiness is contained in 49 U.S.C. § 44704 as implemented by 14 CFR Part 21.
Purpose: The purpose for the collection of this information is to evaluate an applicant’s application
for an Export Certificate of Airworthiness.
Routine Uses: In accordance with the Privacy Act System of Records Notice (SORN), DOT/FAA
801, Aircraft Registration System, 88 FR 53951 - August 9, 2023 will be routinely disclosed:
• To the public (including government entities, title companies, financial institutions, international
organizations, and others), when permitted, information, including aircraft owner’s name, address,
United States Registration Number, aircraft type, legal documents related to title or financing of an
aircraft and ADB–S summary reports, in order to facilitate aviation safety, security, and commerce.
Emails and telephone numbers of small unmanned aircraft system (sUAS) owners registered
under 14 CFR part 48 will not be disclosed to the Public pursuant to this Routine Use.
• To law enforcement, when necessary and relevant to an FAA enforcement activity.
• To government agencies, whether Federal, State, Tribal, local or foreign, information necessary
or relevant to an investigation of a violation or potential violation of law, whether civil, criminal, or
regulatory, that the agency is charged with investigating or enforcing; as well as, to government
agencies, whether Federal, State, or local responsible for threat detection in connection with critical
infrastructure protection.
However, the three routine uses above are only some, but not all, of the published routine uses
that are listed in SORN DOT/FAA 801, Aircraft Registration System. Other general routine uses
noted in the SORN may be applicable as well.
Disclosure: Submission of this information is voluntary; however, incomplete submission may
result in delay or denial of your request.
Paperwork Reduction Act Burden Statement: A federal agency may not conduct or sponsor, and a
person is not required to respond to, nor shall a person be subject to a penalty for failure to comply with
a collection of information subject to the requirements of the Paperwork Reduction Act unless that
collection of information displays a currently valid OMB Control Number. The OMB Control Number for
this information collection is 2120-0552. Public reporting for this collection of information is estimated to
be approximately 12 minutes per response, including the time for reviewing instructions, searching
existing data sources, gathering and maintaining the data needed, and completing and reviewing the
collection of information. All responses to this collection of information are required to obtain or retain

Instructions for Completing FAA 8120-11, Suspected Unapproved Parts Report

Record the date the part was discovered.
Record the part name (or a description of the part).
Record the part number or identification number of the part.
Record the serial number on the part, if applicable.
Record the quantity of parts.
Record the assembly name and assembly number (where the part was or could be installed).
Record additional part numbers on page 3 or on a blank sheet of paper with the same information. Example:

7. Record the type of aircraft the part was (or could be) installed on.
8. Record the complete name and address of the company or person who produced, repaired, and/or sold the part. Do
not list a P.O. Box address unless a street address is not available.
Check the box that describes the company or person and provide the certificate number, if known.
Air Carrier - An FAA-certificated company or person
who undertakes directly by lease, or other
arrangement, to engage in air transportation.
Mechanic - A person holding an FAA mechanic
certificate with airframe and/or powerplant ratings.
Repair Station - An FAA-certificated repair station.
Distributor - A broker, dealer, reseller or other
person or agency engaged in the sale of parts.
Owner/Operator - The owner or operator of an
aircraft.
Supplier - A company or person who furnishes aircraft
parts or related services, at any tier, to the producer of a
product or part thereof.
Production Approval Holder - A company or person
holding one of the following four types of FAA production
approvals: production certificate, approved production
inspection system, parts manufacturer approval, or
technical standard order authorization.
Manufacturer - The original equipment manufacturer
(OEM.)
Other - Record other type of business.

9. Record a brief narrative stating why you believe the part is not approved. Include a description of the part (improper
configuration, suspect marking, different material, etc.), where it was obtained, and what type of documentation was
supplied with it.
10. Record the complete name and address of the location where the part was found. Check the appropriate block to
reflect the affiliation of the company or person who discovered the part.
11. Record the date the FAA 8120-11 is being submitted.
12. Check this box if you request anonymity (do not wish to provide your identity), and do not complete 13 or 14.
13. Record your name, address and phone number, if desired. This information will enable the FAA to contact you for
additional information, if necessary.
14. Check this box if you request confidentiality of your personal information recorded in block 13.

available elsewhere and necessary to support the FAA’s commitment to promote safety. Information is routinely shared with law enforcement
