Thank you for participating in the National Animal Health Monitoring System (NAHMS) Equine
2026 study. The NAHMS staff would like to receive input from you regarding your participation.
Please complete this short online survey within 10 days of completing the event questionnaire. For events
that also agree to an event site visit, please complete this evaluation once the site visit has been
conducted and the event questionnaire has been completed. We value your opinion, so please take a
few minutes to complete this evaluation.
