The Department is submitting a non-substantive change revising Form DS-3025 (Vaccination Documentation Worksheet) to remove the COVID-19 vaccination requirement.
This is in response the updated technical guidance issued by the U.S. Centers for Disease Control and Prevention (CDC), which states, “Effective March 11, 2025, the COVID-19 vaccine will no longer be required as part of the US immigrant overseas medical examination.” The revised form (attached) removed the COVID-19 vaccine line.
