Information Needed Before the Call
To confirm a sampled person’s identity over the phone, you should obtain their information before
calling the sampled person. When you contact the sampled person, you will ask them to provide
matching information. Use two or more methods to confirm their identity.
Examples include:
• Full first, middle, and last name (including suffix, if available)
• Date of birth (month, day, and year)
• Medical facility used
• Phone number
• Current or past address
• Social Security Number
In addition, you should check whether the sampled person is aware of their HIV status by looking
into eHARS or other supplementary sources of information. For example, a single report in eHARS
with no subsequent visits to an HIV provider and no additional CD4 or viral load tests may indicate
that the person may not be aware of their status.

Your health department and the Centers for Disease Control and Prevention (CDC) are doing this
project to learn more about people living with HIV, including the services they use and need. This
information will help improve programs that keep people healthy and get them the help they need.
If you choose to be in this project, we will ask you questions and look at your medical records.
Answering the questions will take about 40 minutes. You don’t have to answer any question you
don’t want to answer. The questions ask about your:
• medical past
• use of medical and social services
• sex practices
• use of drugs and alcohol
• reproductive history
• ability to work and take care of yourself
If we need more information, a staff member may contact you later.
We send the answers to CDC, but we don’t send them your name. Instead, we will assign a code
number to your answers. We don’t send CDC any information that identifies you. We don’t send
CDC any information that could trace back to you. Your answers are confidential. All project
materials are kept in a locked cabinet or secure computer.
