Format: Virtual/Online

Zoom) with a call-in
option
Compensation: $25 gift
card for participating

To Participate,
You Must:
Be a parent/caregiver
who has sought and/or
received behavioral
health care for your
child/adolescent ages
0-21
Be 18 years old or older
Have access to a phone,
computer, or tablet
