VS Form 10-4, Specimen Submission
APHIS conducted a review of VS Form 10-4, Specimen Submission, and requests approval for the following nonsubstantive changes. The changes do not affect activity response time or IC burden.
The data fields were rearranged and renumbered to improve data transcription when processed by the agency.
The previous field, “2. NVSL Submitter ID”, is removed. The previous field, “12. Authorized By”, is removed. The previous field, “14. Country of Origin/Destination” was split into “9. Country of Origin” and “10. Country of Destination”. A new field, “8. FAD Investigation Number”, is added.
The field “Purpose of Submission” is renumbered from 13 to 7, and the 11 selection check boxes renamed from Interstate Movement, Export, Pre-Import, Import, FAD/EP Diagnostic, Surveillance, TB, General Diagnostic, Developmental Research, Regent Evaluation, and NVSL Intralab to 12 selection check boxes named Sick Animal, FAD/EP Diagnostic, Confirmation, Environmental Monitoring, Animal Import/Export, Pre-Import to U.S., Animal Product Import/Export, Interstate Movement, Reagent Evaluation, Agent Characterization, Research, and Healthy Animal (not otherwise specified).
The field “Species or Source” is renumbered from 19 to 11, and renamed “Species or Source of Samples”. Three selection check boxes were added for “Elephant”, “Crustacean”, and “Isolate”.
The field “Specimens Submitted” is renumbered from 17 to 16, and renamed “Sample Types Submitted”. The field “Extract” is replaced with “Trunk Wash”.
The instructions were updated to reflect the above changes.
