Dear Valued SVI Community Member,
We are seeking feedback on the Centers for Disease Control and Prevention (CDC) and Agency for Toxic Substances and Disease Registry’s (ATSDR) Social Vulnerability Index (SVI). The SVI is a place-based index, database, and mapping application designed to identify and quantify communities experiencing social vulnerability.
Your input is vital to help us understand current data needs, who is using the SVI, and how the index and its resources are contributing to planning and decision-making. To that end, we invite you to participate in a brief survey, which should take about 10 minutes to complete. If you work with colleagues who also use the SVI or would benefit from this opportunity, please feel free to share this email with them.
Your responses to this survey are voluntary and we will not ask for any information that will identify you. We hope that you choose to provide feedback. The information gained will assist us in improving the Social Vulnerability Index to support you and the communities we serve.
