Subject: Provide Feedback on the Science Ambassador Fellowship

Thank you for participating in the CDC Science Ambassador Fellowship! We are interested in collecting honest and constructive feedback about your experience this past year. We will be conducting both a brief survey and a 1-hour focus group to collect feedback to help us improve the program in the future. This email contains instructions for how to participate in both.

If you are interested in participating in the survey, please click the link below. The survey takes an average of 14 minutes to complete. You can edit previous responses as you complete the survey. To return to a previous page, use the “Previous” button at the bottom of the page (NOT the “Back” button on your browser menu). To advance, use the “Next” button at the bottom of the page. You will not be able to edit your response when you exit the survey, so please plan to complete this survey in one sitting. The survey will close on June 28 at 11:59 pm ET.

To keep your participation in the survey anonymous, we have created a separate link to indicate your interest in participating in a focus group. The focus groups will be one hour in length and all current fellows are invited to participate.

If you are interested, please share your upcoming availability with our facilitators using the link below by June 27 at 11:59 pm ET. It takes less than one minute to complete the availability poll. You will receive a calendar invitation for a Teams meeting if you are available to participate during the time(s) selected.
