A Census Bureau interviewer, <>, visited your home to ask for your participation in the
National Health Interview Survey (NHIS). Here is more information about this survey.
The Census Bureau has conducted this survey since 1957 for the National Center for Health Statistics
(NCHS), part of the Centers for Disease Control and Prevention (CDC). The survey was created under
Title 42, United States Code, section 242K to report on the health of the nation. It asks about doctor
visits, medical conditions, health insurance, physical activity, and injuries. This information is needed for
health research and planning for future health services.
We pick a sample of addresses to represent all areas of the U.S. Your household represents thousands
of other houses. We cannot replace your address with another. Your input is vital.
We take your privacy seriously. Your answers are combined with those of all other respondents and used
only for statistical purposes. NCHS and its agents, which includes the Census Bureau, will not disclose
any information about you or your household. You also do not have to answer any questions you do not
want to.
We hope you will be able to give us some of your time to answer our questions. Your participation in this

As authorized by Title 13, sections 141, 193, and 221 of the United States Code, the U.S. Census
Bureau conducts surveys each month to provide statistics on topics such as unemployment, education,
income, health, and housing. Title 13 includes a provision that addresses the assistance of property
managers in completing this work.
Surveys conducted by the Census Bureau include the occupants of randomly selected living quarters
throughout the country. The property units at <> are part of an official
survey sample. Our Census interviewer, <>, needs to speak with residents of sampled
households within the property until their work is completed. The interviewers are official representatives
of the U.S. Department of Commerce, Bureau of the Census. They have official identification cards in their
possession.
