The information on this form is required to request a refund from the American Pecan Promotion Board (APPB) for the payment of assessments from an escrow account during the period beginning on the effective date of the Order and ending on the date the results of the referendum are announced as provided in 7 CFR 1223.57.
INSTRUCTIONS: 1. This application must be postmarked within 60 days after the announcement of the referendum. 2. Attach documentation or a copy thereof, or such evidence deemed satisfactory to the Board, with this application. 3. Applicant’s name must be the same as it appears on the bill of sale or other proof of sale. 4. Refund application must be signed by the person requesting the refund.

The time required to complete this information collection is estimated to average 30 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.
