will report data to MIPS.
• If you are reporting data at the individual level, complete the hardship exception
application at the individual level.
• If you are reporting data at the group level, complete the hardship exception application
at the group level.
• Note, a group hardship exception application will only apply at the group level.

• If your practice is participating in MIPS at the individual level, don’t complete the
