PHAP Alumni Survey
Introduction
Thank you for taking the time to participate in the PHAP Alumni Assessment survey. The
purpose of this survey is to learn about your career progression, accomplishments, and
perceptions of PHAP's value.
All information that you provide will be kept secure and not shared. This survey is voluntary and
will have no negative effect on you if you decide not to participate. Results of this survey will
only be reported in the aggregate (e.g., 75% of alumni are employed in public health). The
survey should take you fewer than 10 minutes to complete.

The public reporting burden of this collection of information is estimated to average 8 minutes per response,
including time for reviewing instructions, searching existing data sources, gathering, and maintaining the data
