CLOSE INDIVIDUAL FISHING QUOTA (IFQ) ACCOUNT REQUEST
Instructions: Please complete this form if you want to close your IFQ online account. This form must be
completed and signed by all holders named on the IFQ shareholder or dealer account. If an IFQ account
holder is deceased, the person requesting the closure must sign the form, indicating the relationship to the
deceased; provide a death certificate and any additional information NOAA Fisheries Service determines is
necessary to complete the request. Shareholder accounts cannot be closed until all shares and allocation
have been transferred to another IFQ account. Dealer accounts cannot be closed until all cost recovery
fees have been paid. Please complete and mail this form to:
