INSTRUCTIONS
1. Enter name of vessel on which the scale test is conducted.
2. Enter date of test.
3. Enter time of test.
4. Enter number of sand bags used for each run of the test.
5. Weigh sandbags on Platform scale (A). Weigh no more than 50 kg of bags per load.
6. Record the number of times the sand bags were run across the scale (B). Each run can be no less an 40 kg of bags.
7. Multiply the total weight of the sand bags on the platform scale by the number of times the sand bags are run (AXB=C).
8. Record the weight of sand bags run across the flow scale for all runs (D).
9. Record the flow scale error. Subtract platform scale weight from flow scale weight (D-C=E).
10. Record the percent error of the flow scale. Divide Error by platform scale weight & multiply by 100 (E÷Cx100).
11. Record whether the scale passed or failed the daily test. If the percent error is between -3.0 % and +3.0% the scale passes.
12. Record name(s) of person(s) conducting the test.
13. Record the cruise number of the observer that witnessed the test.
14. Vessel operator must sign the form.

DAILY FLOW SCALE TEST ADDITIONAL INFORMATION
 A daily scale test must be conducted once each calendar day, not to exceed 24 hours, when the scale is being used to weigh catch atsea.
 Record all tests including failed tests.
 If the scale fails the daily test, it may be re-tested at any time. However, it may not be used to weigh fish until it passes the daily test.
 This form must be maintained on board the vessel until the end of the fishing year in which it was completed. It must be retained by
the vessel owner for three years, and must be made available to NMFS personnel, observers or authorized officers when requested.

PRIVACY ACT STATEMENT
Authority: The collection of this information is authorized under the Magnuson-Stevens Fishery Conservation and Management Act, 16 U.S.C
1801 et seq.
Purpose: In order to manage U.S. fisheries, the NOAA National Marine Fisheries Service collects permit holder information forms for the At-Sea
Scales Program, which was developed in response to the need for catch accounting methods that were more precise and verifiable at the level of
the individual haul and less dependent on estimates generated by at-sea observers. This was necessary due to the implementation of large-scale
catch share programs that required NMFS to provide verifiable and defensible estimates of quota harvest. NMFS also established a set of
performance based standards to allow shoreside processors to describe how they will ensure that all delivered catch can be effectively monitored
by NMFS-authorized personnel, and that all catch is accurately sorted and weighed by species.
Routine Uses: The Department will use this information to identify fishery participants and evaluate the qualifications of the applicants.
Disclosure of this information is permitted under the Privacy Act of 1974 (5U.S.C. Section 552a), to be shared within NMFS offices, in order to
coordinate monitoring and management of sustainability of fisheries and protected resources, as well as with the applicable State or Regional
Marine Fisheries Commissions and International Organizations. Disclosure of this information is also subject to all of the published routine uses
as identified in the Privacy Act System of Records Notice COMMERCE/NOAA-19, Permits and Registrations for the United States Federally
Regulated Fisheries.
Disclosure: Furnishing this information is required to obtain or retain the benefit of participation in either an at-sea scales program or shorebased catch-monitoring program; failure to provide complete and accurate information will prevent the determination of eligibility for registration in
either of these programs.
