Re: Thank-you and follow-up document request for FNS SNAP Work Requirements and E&T Data Study

Dear [First Name] [Last Name],

Thank you for taking the time to complete the SNAP Work Requirements and E&T Data Study survey. Your response helps provide important information about measures and data FNS could use to better understand equity in the administration of work requirements in SNAP, including access to and administration of SNAP E&T services.

If you have not already submitted your documents, please upload them to your State’s unique Box folder, a secure cloud-based storage platform, using this link:

[Box link here]

We have also attached a guide that includes a table identifying the types of documents (i.e., Category) and examples we would like you to provide. This guide is also in your State Box folder. Please fill out the guide with the documents you have uploaded.

If you have already submitted all of your SNAP Work Requirement and E&T documents, thank you.

If you have any questions, please reply to this email or contact us at [email]. Your FNS point of contact for this survey is Kristen Corey (kristen.corey@usda.gov).

We sincerely appreciate your participation and look forward to receiving your documents!

Sincerely,

[Westat POC]
