Participant Information Sheet & Consent
Homeowner Perceptions, Barriers, and Intentions to Install a Living Shoreline
Researcher: Dr. Blake Simmons, Tampa Bay Estuary Program

You are invited to complete a questionnaire about your perceptions of living shorelines.
Purpose
The purpose of this research is to collect crowdsourced data from homeowners in the Don CeSar neighborhood about
their opinions of living shorelines and their interest in installing living shorelines on their property. The results will be used
to identify key barriers, motivations, and other important factors that influence the adoption of living shorelines.
What is involved?
If you agree to participate, you will be asked to complete a short questionnaire before and after today’s presentation. This
should take about 5-7 minutes. After this is completed, we will send you three short follow-up questionnaires over the next
12 months to track how your interests and opinions of living shorelines might change over time.
Privacy and confidentiality
To match your answers to the questionnaire today with your answers in future questionnaires, we will ask you for your
property address and/or email address. The lead researcher, Dr. Blake Simmons, is the only person who will have access
to this information, and we will never publish or share your contact information with anyone. Once the study is complete,
your contact information will be permanently redacted and deleted from the records to ensure your responses remain
anonymous and no one will be able to identify you.
Is there likely to be a benefit to me?
We plan to use this data to better understand what is important to homeowners when considering installing a living
shoreline on their property. We can’t guarantee that you get any direct benefit, but the results of this research will help us
design better programs to increase shoreline enhancements across Tampa Bay.
Participation in this research is entirely voluntary. You can stop at any time without explanation. There will be no
penalties or loss of benefits to you if you do not participate or decide to stop once you start.
If you would like more information about the research, contact Dr. Blake Simmons: bsimmons@tbep.org
This collection of information is approved by OMB under the Paperwork Reduction Act, 44 U.S.C. 3501 et seq. (OMB
Control No. 2030-0051). Responses to this collection of information are voluntary. An agency may not conduct or sponsor,
and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control
number. The public reporting and recordkeeping burden for this collection of information is estimated to be 10 minutes per
response. Send comments on the Agency’s need for this information, the accuracy of the provided burden estimates and
any suggested methods for minimizing respondent burden to Director, Information Engagement Division, U.S.
Environmental Protection Agency (2821T), 1200 Pennsylvania Ave., NW, Washington, D.C. 20460. Include the OMB
control number in any correspondence. Do not send the completed form to this address.
Consent
If you understand these issues, would like to participate in this research, and are over 18 years old, please select ‘I agree’
