JumpStart Questions for CHC Survey
The following module seeks to assess the use of the American Heart Association
(AHA)/American Medical Association (AMA) JumpStart curriculum for Self-Measured Blood
Pressure Monitoring by participating NHCI health centers. Questions will cover the following
topics:

Access and use of the JumpStart curriculum for SMBP programs;
Usefulness and relevancy of the JumpStart curriculum for SMBP programs; and
Helpfulness of the JumpStart curriculum in SMBP implementation and adoption of best
practices

We recommend that survey respondents include staff who have logged onto the JumpStart
curriculum in responding to and reviewing these questions.
