Receiving handler must check applicable sections in Part B, sign, and then mail or fax a copy of this form to the transferring handler and the ABC. This report is submitted to adjust assessment and reserve obligations and for use in redetermination reports, in compliance with sections 981.55 and 981.61 of Marketing Order No. 981.
