NMDPSM Donation Experience Survey



NMDP is conducting a survey to better understand your donation experience – specifically the few days prior to your donation, your donation day, and early recovery. The survey takes 5-10 minutes to complete. Thank you for sharing your feedback and helping us improve.

Public Burden Statement: As the contractor for the C.W. Bill Young Cell Transplantation Program Single Point of Access – Coordinating Center, the National Marrow Donor Program d/b/a NMDPSM (“NMDP”) is required to conduct surveys to evaluate satisfaction with the services provided. NMDP will elicit feedback from blood stem cell donors to better understand their overall experience and satisfaction. The results of this survey will be used to identify trends and opportunities for improvement. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The OMB control number for this information collection is 0915-0212 and it is valid until XX/XX/202X. This information collection is voluntary. Collection of this information fully complies with the Guidelines of 5 CFR 1320.5. The public reporting burden for this collection of information is estimated to average .13 hours (8 minutes) per response, including the time for reviewing instructions, searching existing data sources, and completing and reviewing the collection of information.
