Dear NPDB User,
The U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) is conducting a survey of eligible users of the National Practitioner Data Bank (NPDB). Periodically, we conduct surveys to ensure that we meet the needs of our users. Your input is valuable and will be instrumental in guiding the NPDB's future initiatives. We appreciate your willingness to participate.
To conduct this survey, we have partnered with a small business, InfoStatica Inc. You have been chosen to offer insights regarding querying the NPDB. If you aren't directly responsible for querying practitioners, we kindly ask that you pass this along to the appropriate person in your organization.

Public Burden Statement: The purpose of this survey is to evaluate NPDB query forms to determine whether there is a need to collect new information or discontinue collecting information that is not useful. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The OMB control number for this information collection is 0915-0212 and it is valid until 4/30/2024. This information collection is voluntary. Public reporting burden for this collection of information is estimated to average 10 minutes per response, including the time for reviewing instructions, searching existing data sources, and completing and reviewing the collection of information.
