As a Kodiak Island Roaded Area Registration Permit holder, you are required to complete and return
by mail the permit report, even if you did not harvest birds or eggs. Your responses are confidential.
You are receiving this reminder because we have not yet received your report. To avoid future
notifications, please complete and return this report now. Harvest estimates are used to:
Document the importance of subsistence uses of migratory birds. Protect sustainable subsistence
harvests. Protect our bird resources. Assess whether harvest regulations are appropriate.
Instructions for completing your report:
1. Each individual permit holder must complete her/his
own report. If more than one person in a household
obtained a permit, each person must answer
her/his individual report.
2. Write your permit number and date in the white
boxes at the top of the first report page.
3. For each season, checkmark in the “YES” or “NO”
white circle indicating whether you harvested birds
and/or eggs. In the fields provided close to bird
drawings, report numbers of all birds and eggs you
harvested, including those that you gave to other
people.
4. If you harvested with other people, report only your
share of the harvest.
5. Do not report birds or eggs you harvested in other places (outside the roaded area) and times of
the year.
6. Do not report birds or eggs received from other people.
7. Report numbers of birds and eggs as individual units. For instance, if you harvested eggs using a
5-galon bucket or other kind of container, specify how many eggs.
8. Write comments in the box provided at the bottom of each seasonal report sheet (weather,
hunting conditions, access to hunting areas, unusual birds seen, permit and reporting process,
etc.).
9. Fold together the two report sheets, put them in the pre-stamped envelope provided, and mail it to
the pre-printed address.

Paperwork Reduction Act Statement
In accordance with the Paperwork Reduction Act (44 U.S.C. 3501 et seq.), the U.S. Fish and Wildlife Service (Service)
collects information authorized by the Migratory Bird Treaty Act (16 U.S.C. 703 et seq.), the Migratory Bird Treaty Act
Protocol Amendment (1995), and its letter of submittal from the Department of State to the White House, which specifies
the need for harvest monitoring. Information requested in this form is purely voluntary. We will use the information your
household provides to estimate subsistence migratory bird harvest in subsistence eligible areas of Alaska. Household
harvest reports are anonymous and we do not collect names on harvest report forms. According to the Paperwork
Reduction Act of 1995, an agency may not conduct or sponsor and a person is not required to respond to a collection of
information unless it displays a currently valid OMB control number. OMB has approved this collection of information and
assigned Control No. 1018-0124.
Estimated Burden Statement
With help of a surveyor, we estimate it will take about 5 minutes each to provide household consent and to report your
seasonal bird/egg harvest, including time for reviewing instructions, gathering and maintaining data and completing and
