Subject Line: NCCIH Diversity Supplement Feedback Survey

Dear Dr.

The National Center for Complementary and Integrative Health (NCCIH) would like to thank you for being a previous Diversity Supplement Scholar. NCCIH is strongly committed to supporting the development of a diverse and well-trained research workforce through use of diversity supplements. 

To help us improve our Diversity Supplement Scholar program for future scholars we have developed a survey to help us identify strengths/weaknesses. Based on your experience as a Diversity Supplement Scholar, we would appreciate it if you would please complete our survey. Please note that no personal identifiers are being collected so that all responses will remain anonymous. We anticipate that the survey will take approximately 10 minutes to complete and will be available for the next 3 weeks until <insert date>.
