Description
This data collection does not involve collection of sensitive and/or personally identifiable information. Respondents
are state-, territorial/USAPIJ-, and tribal-based comprehensive cancer control programs. Although contact
information is obtained for each program, the contact person provides information about the state, territorial, or
tribal program, not personal information.
NCCDPHP's Award Management Platform (AMP) (OMB Control 0920-1132) is used to manage the technical assistance
to the grantees. The proposed data collection instruments include: (1) REDCAP web-based survey to be disseminated
in years 02 and 04 targeting program directors and coalition partners and (2) In-depth interviews (IDIs) of CCC
program implementers using a case study approach and executed during years 1-5. The proposed data collection
instruments will ensure collection of previously collected data via the Chronic Disease Management Information
System (CDMIS) which are not collected in the Award Management Platform (AMP). CDC will continue to have
administrative, technical, and physical controls in place to protect the data collected from the proposed collection
instruments. Under this information collection request, we will not collect personal identifiable information or utilize
an IT database to store such information.
Data is cut off to closed grant, contract, or cooperative agreement files at the end of the calendar year in which the
project ends or a final report is written. Maintain in office two years after cut off. Transfer to FRC and destroy six
years after cut off.
