What is the Purpose of Form G-1579?
Form G-1579, Outstanding Americans by Choice, Nominee Questionnaire, is used to verify the naturalization information
of an Outstanding Americans By Choice nominee, describe the functions and duties of Americans by Choice recipients,
assess nominees’ willingness to participate, and obtain consent to be recognized as an Outstanding Americans by Choice
recipient.

Who May Use Form G-1579?

Individuals contacted by U.S. Citizenship and Immigration Services (USCIS) who consent to be considered for selection
as an Outstanding Americans by Choice (ABC) recipient.

General Instructions

Accommodations. USCIS is committed to providing disability accommodations to individuals selected as Outstanding
Americans by Choice recipients and any applicable guests. To request an accommodation, please email your request to
Outstanding-abc@uscis.dhs.gov.
Signature. Each questionnaire must be properly signed and filed. For all signatures on this questionnaire, USCIS will
not accept a stamped or typewritten name in place of a signature. If you are under 14 years of age, your parent or legal
guardian may sign the application on your behalf. A legal guardian may also sign for a mentally incompetent person.
Validity of Signatures. USCIS will consider a photocopied, faxed, or scanned copy of the original handwritten signature
valid for filing purposes. The photocopy, fax, or scan must be of the original document containing the handwritten ink
signature.

ABC Program Notification Process
If you are qualified for this program, you will receive a notice titled “Confirmation of Selection as Nominee for
Outstanding Americans by Choice Initiative.” Form G-1579A, Outstanding Americans by Choice Supplemental
Questionnaire, will be attached to this notice. The information collected on this form allows USCIS to coordinate your
attendance and remarks in connection with the event where you will be publicly recognized. Please complete this form
and return it to us via Email at Outstanding-abc@uscis.dhs.gov. For more information, please refer to the notice.
If you are not qualified for this program, you will receive notice of our decision through an Email sent to the Email
address you provided.

Proof of Naturalization: (Part 1., Item Numbers 1. - 3.)
We must confirm that you are a naturalized U.S. citizen. If you are a naturalized U.S. citizen, you may provide a scanned
copy of your Certificate of Naturalization by Email, or consent to the use of USCIS internal databases to confirm your
naturalization status.
Certificates of Naturalization may be Emailed to USCIS at Outstanding-abc@uscis.dhs.gov. We recommend that
you encrypt the email before sending. NOTE: This secure government mailbox is used solely for communication with
ABC candidates. For more information about the Outstanding Americans by Choice initiative, visit www.uscis.gov/

DHS Privacy Notice

AUTHORITIES: The information requested on this form, and the associated evidence, is collected pursuant to Section
8 U.S.C. 1443(h) of the Immigration and Nationality Act (INA).
PURPOSE: The primary purpose for providing the requested information on this questionnaire is to determine if you are
eligible for recognition as an Outstanding American By Choice recipient. DHS uses this information to confirm that you
are a naturalized citizen and that you consent to be recognized as an Outstanding American By Choice recipient.
DISCLOSURE: The information you provide is voluntary. However, failure to provide the requested information may
delay your recognition as an Outstanding Americans by Choice recipient or result in the closing of your nomination as an
Outstanding Americans by Choice recipient.
ROUTINE USES: The information will be used by and disclosed to Department of Homeland Security personnel and
contractors or other agents who need the information to assist in activities relating to the selection of a nominee for the
Outstanding Americans by Choice recognition. USCIS may seek information pertaining to your naturalization status
to determine eligibility. Failure to provide may prevent USCIS for determining your naturalization status and being
eligible to receive the recognition. The information will be used in accordance with approved routine uses, as described
in the associated published system of records notices [DHS/ALL-011 - Department of Homeland Security Biographies
and Awards, DHS/ALL-024 Department of Homeland Security Facility and Perimeter Access Control and Visitor
Management, and DHS/USCIS-007 Benefits Information System], which can be found at www.dhs.gov/privacy. DHS
may also share this information, as appropriate, for law enforcement purposes or in the interest of national security.
