Collection of nonfuel minerals information is authorized by Public Law 96-479 and the Defense Production Act. This information is used to support executive policy decisions
pertaining to emergency preparedness, national defense, and analyses for minerals legislation and industrial trends. The USGS relies on your voluntary and timely response
to assure that its information is complete and accurate.

Please complete and return a separate form WITHIN 3 WEEKS for EACH OPERATION in which you produced fertilizer materals during the 6-month report
period in the enclosed envelope or fax to the above toll-free number. If you do not have exact data, please enter your best estimates and mark with an /e.
Use zero (0) when appropriate. Do not report decimals or fractions. Round to the nearest whole number. If you have nothing to report, please sign and
return the form. Please do not make entries in shaded areas. THIS REPORT IS INTENDED TO COLLECT DATA ON NITROGEN AND PHOSPHATE FERTILIZER
MATERIALS FROM THE PRODUCERS OF THESE COMMODITIES. Please use the space for "Remarks" to provide any specific information that will help us in

SPECIFIC INSTRUCTIONS
Ammonia, synthetic, anhydrous - Report total production of ammonia whether used or shipped as anhydrous or converted to aqua. This value should
represent total ammonia leaving compressors and not calculated from the NH3 content of the finished products.
Ammonium nitrate - Report total production of the ammonium nitrate content of original melt liquor chemically synthesized from ammonia and nitric
acid, including amounts produced for fertilizer, explosives, and other uses, and amounts consumed in manufacturing other products. Stocks represent
total stocks held by producing companies, including stock of original melt liquor and amounts (liquid and solid) reported as fertilizer, explosives, and
other uses.
Urea - Report total production of urea content of original melt liquor synthesized from ammonia and carbon dioxide, including that produced as an
intermediate product.
Normal and enriched superphosphates - Include all grades containing less than 40 percent available phosphoric oxide (P2O5).

MAP (monoammonium phosphates) - Include chemically processed nitrogen-phosphorus materials containing 50 percent or more primary nutrients and
having a phosphate to nitrogen ratio of 3 to 1 or greater.
DAP (diammonium phosphates) - Include chemically processed nitrogen-phosphorus materials comprising fertilizer grades containing 18 to 21 percent
nitrogen (N), and 46 to 54 percent phosphoric oxide (P2O5).
