To establish the marital relationship please complete Form G-124 and have your marriage partner
(if alive) complete a separate Form G-124.
You must also have one of your blood relatives complete Form G-124a and your marriage partner
must have a blood relative complete a Form G-124a.
If the G-124a cannot be obtained from a blood relative of one party, it should be obtained from a
relative of the other party. If a G-124a cannot be obtained from blood relatives of either party, you
must furnish this office with a written explanation why it could not be obtained.
If, however, you feel that securing the Form G-124a from a blood relative would be detrimental to
you, your marriage partner, or any children born of the marriage, then you must furnish this office
with a detailed explanation, in writing, of why it would be detrimental.
Please know that all information furnished on Forms G-124 and G-124a, and in documentary
evidence submitted to establish the marital relationship, is confidential and protected by the
Privacy Act (see detailed explanation on the forms).
