GENERAL: This information is being collected for the primary purpose of collecting information from a
local government to apply for the cancellation of a FEMA Public Assistance Division Community Disaster
Loan (CDL).
AUTHORITY: Section 417 of the Robert T. Stafford Disaster Relief and Emergency Assistance Act, as
amended (42 U.S.C. § 5183).
USES AND SHARING: Application for CDL Loan Cancellation Forms are used by local governments to
comply with important Federal requirements and to apply for the cancellation of a FEMA Public
Assistance Division Community Disaster Loan (CDL). FEMA may share the information collected through
these forms with the FEMA Finance Center in order to establish the applicant’s loan account.
EFFECTS OF NONDISCLOSURE: The disclosure of information on this form is voluntary; however, failure
to provide the information requested may delay or prevent FEMA from providing the requested funding.
