NATIONAL HEALTH INTERVIEW SURVEY (NHIS)
An Opportunity for You
Over the years, thousands of families across
your community, state, and country have helped
the nation by participating in this survey. Now is
your chance to help.
What Is NHIS?
NHIS is the primary source of information on
the nation’s health. As the country’s largest
in-person household health survey, NHIS has
been taking the pulse of the nation since 1957.
Each Selected Household
Is Important
The chances of being selected for this survey
are small. Once selected, each household
represents thousands of others. Your household
is irreplaceable.
We are happy to fit
into your schedule.
Interviews can be
conducted at a
time convenient
for you.
To make an appointment to
participate in NHIS, call the phone
number on the letter you received.
