NEA Jazz Masters Fellowships Nomination Form
To make a nomination, enter the first and last name of the individual and indicate if the
nomination is for the performer or advocate category. To complete the nomination, you
must attach the two required PDF supporting documents as described in the guidelines,
and then click submit.
A nominee must be a living U.S. Citizen or permanent resident of the United States.
Nominations must be submitted in English.
