Request for Information Instructions
Personal Information
Last name and first name are required entries. Please provide legal names (no nicknames or initials).
Citizenship Type: Utilize the pull-down menu to select the appropriate response.
Use the following guidelines for completing Citizenship Type:
Born in the United States or born overseas to U.S. citizens, select Native Born American.
Became a U.S. citizen since birth, select Naturalized Citizen.
Legally immigrated to the U.S., select Immigrant Alien.
If you are not a U.S. citizen and do not live in the U.S., do not fill out this application. International
students must work with the American Embassy in their home country to begin the application process.
By law, the Naval Academy is authorized to accept up to 60 international students officially sponsored by
their respective governments.
Birth Date and Social Security Number: A valid birth date OR Social Security number are required
entries. You do not have to provide both. This information is used by the Naval Academy Admissions
Office to create informational files for record and needed to prevent duplicate information entry. For birth
date, enter information in the format of DD-MON-YYYY where DD is the two-digit date, MON is the first
three letters of the month, and YYYY is the four-digit year. You may also select the date from the
adjacent calendar link.
Race and Ethnicity: You may decline to respond to these cues, but the Naval Academy requests that
you answer these as accurately as possible.

Contact Information: Enter your CURRENT MAILING address information accurately. Mailed
information will be sent to this address and future information will also go to this address. Enter only
address information (Do not re-enter your name).
If you have already filled out a Request for Information and are trying to change or update your current
address, contact the Office of Admissions at (410) 293-1858 or email us

Email Address: A valid email address is a required entry to complete this form. If you do not have an
email address or have access to one, create an account through your school or library. Additionally,
there are a number of internet providers that offer free email services.
Phone Number: Enter a valid phone number to be used by Admissions personnel in order to contact
you in the case of an admissions event taking place in your area. PLEASE ENTER ONLY NUMERIC
CHARACTERS.
Year of High School Graduation: Enter the year in which you anticipate graduating from high school or
did graduate.
High School Name: This is not a required entry, but it will help the Naval Academy put you in contact
with a local representative. You must use the "Select From List" link, then enter the city in which your
high school is located when cued (Do not include the state). If your high school is located in a city with
"Fort", "Saint", or "Mount" in its name, try both the abbreviations (Ft, St, and Mt) and the full versions
when searching. If you are a home-schooled student, enter "home school" in the city search, and make
the appropriate selection from the list. A list of schools in the city you entered will be generated, and you
must select yours from the list. This will populate the application entry. Select the correct school from the
list (Multiple schools with the same name may be listed from the various states).
Sports Camp: Answer ‘yes’ or ‘no if you are interested in attending a sports camp at the United States
Naval Academy. If you answer yes, you will have to answer the following question, providing the type of
camp in which you are interested.
Military Status: If you are on Active Duty in the Armed Forces, select your rank and service by using the
respective pull-down menus.
Ensure that you verify the accuracy of your completed request prior to submitting. When you select
'Submit', the application module will check your data entered for format errors and confirm your desire to
submit. Continue the submission by confirming. A message in green letters that reads 'SUCCESS' will
confirm your successful submission of an application, and a confirmation will be sent to the email
address provided within the form.
