pdfINSTRUCTIONS FOR COMPLETING THE LOGBOOK FORM
Start a new logbook page for every trip. Please fill out a line for each day of fishing effort; each page can be used for up to
4 days of fishing effort.
Trip Section:
vessel name, captain, captain commercial fishing license number, Coast Guard vessel ID or State Registration Number,
observed trip (yes or no), departure port, departure date, arrival port, arrival date.
Landing Section:
port landed, landing receipt number(s), permit type (circle one)
Fishing Effort Section:
FOR EACH INDIVIDUAL DAY ENTER STANDARD (UNLINKED BUOYS) GEAR IN THE TOP WHITE LINE AND LINKED
GEAR IN THE SECOND GRAY LINE. Date of fishing, latitude/longitude in degrees and minutes, number of deployed
hooks and pieces (pieces defined as a single linked or a single unlinked buoy), number of hours that the gear was soaked
in the water, number of fish kept and number of fish released for each corresponding species (if species not listed use
“other” and fill in species name), night-set (circle ‘Y’ if the set was made at night), comments on anything of note. *please
note that only fish caught using deep-set or night-set buoy gear should be recorded here and you must fill out the
appropriate logbooks for your catch from other gears*
SUBMITTING LOGBOOKS
HMS permit holders are required to submit either the original logbook or a copy via email within 30 days after each trip to
the following address:
