OSAC Registry Implementation Survey
The Organization of Scientific Area Committees (OSAC) for Forensic Science wants to better understand
how the standards on the OSAC Registry are being used, the challenges around standards
implementation, and what support is needed to improve it. The OSAC Registry Implementation Survey is
a tool we use to collect this information on an annual basis.

OSAC needs to hear from the nation’s forensic science service providers! Please help us by completing
the 2022 OSAC Registry Implementation Survey which provides an assessment of 95 standards posted
on the OSAC Registry through June 2022.

By entering your email address below, you will receive a copy of your survey response.  Your email
address will remain confidential.

Respondents can prepare for the survey by reviewing a list of the standards represented in this survey
here:   https://www.nist.gov/osac/osac-registry-implementation-survey.

For more information about OSAC visit: https://www.nist.gov/osac.

The OSAC Program Office within the National Institute of Standards and Technology (NIST) Special
Programs Office is managing this survey. If you have any questions about the survey or need assistance
please e-mail forensics@nist.gov.

THE SURVEY WILL CLOSE AUGUST 12, 2022.
ONLY ONE (1) SURVEY RESPONSE SHOULD BE SUBMITTED PER LOCATION (For example, a
state laboratory with multiple regional laboratories should submit a separate response for
each of its locations.)     THE SURVEY WILL TAKE APPROXIMATELY 15-60 MINUTES TO
COMPLETE.  YOUR PARTICIPATION IS VOLUNTARY.
SURVEY INFORMATION SHEET is available for survey participants which explains this study.
You should read the Information Sheet at https://www.nist.gov/osac/osac-registryimplementation-survey.  Upon completion, select the option below that applies.

54.

In addition to this survey, OSAC has developed an Implementation Declaration
Form that FSSPs can complete to inform OSAC of their current compliance or
self-adoption of the applicable standards on the OSAC Registry in their SOPs or
quality manual. A FSSP that has completed and submitted this form is eligible to
receive an OSAC Registry Implementer Certificate.

Should OSAC acknowledge this survey as your self-declaration and send your
organization an OSAC Registry Implementer Certificate?
Mark only one oval.

submit your survey.
No. Thank you for completing the survey. Continue to submit your survey.

Thanks for
participating.

Thanks for volunteering to participate in the OSAC Registry Implementation Survey and
for the time and effort to complete it.  Time is one of the most valuable things we have
and the OSAC Program Office expresses a sincere appreciation for the time given to this
activity.  We are hopeful that we will get clear responses that indicate whether we are on
the correct path to improving implementation of the standards on the OSAC Registry.
