The proposed Form 52671-E will be
completed by property owners who have project-based rental
assistance contracts with HUD, who wish to receive a supplemental
payment to offset operating cost increases to prevent, prepare, and
respond to the effects of COVID-19. Owners of Section 8 Housing
Assistance Payment contracts, Section 202 and Section 811 Project
Rental Assistance contract, Section 202/162 Project Assistance
contracts, and Section 202 Senior Preservation Rental Assistance
contacts.

requesting an expedited approval to utilize DocuSign, a digital
tool as the method for fulfilling the certification requirement for
owners with conditionally approved CSPs, a requirement mentioned in
the currently approved collection: OMB 2502-0619. An explanation of
this tool is as follows..... The creation of the subject DocuSign
template is critical in implementing the funding request process
HUD has established for COVID-19 Supplemental Payments (using HUD
52671-E, in conjunction with Notice H 2021-05 and FAQs dated
10/07/21). In the relatively small number of instances where
capital equipment purchases made by Multifamily properties were
under contract but could not be fully delivered and/or installed
prior to HUD’s CSP Round IV request deadline, HUD is requiring
owners with conditionally approved CSPs to follow-up their HUD
52671-E submission with certification that equipment
delivery/install is complete. As CSP funds are available on a
strictly reimbursable basis, HUD will not disburse funds until this
certification is received. The requirement to submit certification
of completion was indicated on the HUD 52671-E, though the format
and procedures to submit these delayed certifications were not
specified beyond a general reference to use of email. HUD now
anticipates allowing certification through an email/DocuSign
template originated by HUD that will be emailed directly to
impacted owners by HUD to submit certification. This is the most
efficient and least burdensome option for both HUD and the property
owners. We will also use the same email/Docusign template to allow
property owners to request an extended amount of time to complete
delivery/installation of equipment in contexts where vendors are
experiencing unexpected supply chain delays and staffing shortages.
