ASSIGNMENT MESSAGE: Please investigate the attached explosion or fire incident reportedly
involving LP (propane) and NG (natural gas) combustion appliances (Use Appendix 97 – LP Gas Fuel
Train or the product specific guideline (e.g., Furnaces (Natural and Liquefied Petroleum Gas) Appendix
98; Control Panel – Gas Ranges Appendix 138) as appropriate to conduct the IDI. Document all injuries
and collect official reports.
Be sure to obtain:
• Pre-incident, incident, and post-incident scenarios.
• Document any repairs or maintenance conducted on the product, LP or NG supply piping, LG or
propane tank or pressure regulator, NG service regulator, and/or meter set assembly prior to the
incident, including any problems the consumer noticed.
• Document all standards markings and warnings that appear on product.
• Copy of owner’s manual for product.
• Document the fuel supply connection between the manual gas shutoff valve/regulators and the
appliance, if possible. Also, document the LP or NG supply piping, LG or propane tank and
pressure regulator, NG service regulator, and/or meter set assembly.
• If there was a fire, where were the flames coming from? How long was the appliance in
operation prior to the incident?
• Was the consumer attempting to use unit when the fire/explosion occurred, or did it happen after
the appliance had been in use for some time? Was there a controller issues that turned the unit
on/off without an operator present?
• Document the appliance. Obtain photos of appliance, the LP or NG supply piping, LG or
propane tank and pressure regulator, NG service regulator, and/or meter set assembly.
• If there was a fire, how big were the flames?
• Describe any sounds or noises reported prior to, during, or after the incident. Document whether
there was a whooshing or popping sound?
If applicable, please also complete the guideline for Smoke Alarms (Appendix 128).
Please include all primary and all backup contacts in the distribution of the completed IDI.
