APHIS conducted a review of VS Form 1-5, Nomination Request Form, and requests approval for the following nonsubstantive changes.

1. Added a “Clear Form” button top right of first page.
   
   

2. The name of the form was change from Nomination Request Form to Nomination Request Form – Animal Disease Training.
   
   

3. The email address at the bottom of the form was changed from

PROFESSIONAL.DEVELOPMENT.STAFF@APHIS.USDA.GOV

to

APHIS.VS.TRAINING@USDA.GOV.

4. The link to the list of PDS points of contact at the bottom of the form was changed from
